FAQs
Maybe you are new to physical therapy, or it’s been a while. Either way, we’re here
to help. Here are the questions we routinely hear from our patients. If your questions
are not answered here, please contact us.
What do I bring to my first appointment?
- Please bring your physical therapy prescription if your doctor prescribed PT
- Your insurance card if you are using insurance
- Download and complete all three (3) forms below and bring them to your first visit
Patient Health Questionnaire (2 pages)
Payment Policy/ Consent to Treat Form
HIPAA Consent Form
What can I expect at my first appointment?
You can expect a warm welcome! We will perform a thorough evaluation including discussing
your medical history, your current diagnosis and the challenges that come with it,
physical assessment and possible administering of treatment.
How many visits will I need?
The number of visits varies. It depends on your diagnosis, medical history and overall
health. We will periodically re-evaluate you and provide you with information that
you will need to follow up with your doctor. Our goal is to get you to your optimal
health in the safest and most effective way for you.
What do I wear to physical therapy?
First and foremost, wear something comfortable! We recommend loose fitting clothes
so that we have access to the treatment area. For example, shorts are a good idea
for knee problems and tank tops or short-sleeved shirts for shoulder pain.
How long will my appointment last?
The initial appointment will last approximately 60 minutes, with subsequent appointments lasting 45-60 minutes.
